Question 1 sa 3

Best answer: B

Mahalagang cue: Potassium concentrate by direct IV push ang order; wala pang naibibigay.

Best answer: C

  • A. Ibigay ang scheduled dose dahil araw-araw itong iniinom.

Best answer: A

A medication name is only part of the decision. The route, the client's current findings and the exact formulation can change what the nurse should do next. What these three questions teach

1. A valid indication does not make every route safe

The potassium question asks you to notice the administration route before giving any medication. Potassium chloride concentrate must be diluted for an appropriate infusion; direct injection can cause fatal cardiac harm. A slower push or a syringe dilution does not resolve an unsafe IV-push order. Clarify the complete order instead of inventing a different route, dose or rate.

The potassium chloride prescribing information supports this distinction. The example does not prescribe a replacement regimen; actual replacement depends on the client, prescription and local protocol.

2. A routine schedule does not override new findings

In the metoprolol question, the slow pulse, low blood pressure and new dizziness belong together. The decision is to withhold the dose now and seek urgent assessment and monitoring, rather than give it and wait. The AHA adult bradycardia algorithm calls for assessment and support when bradycardia is accompanied by cardiopulmonary compromise, including hypotension. Reducing the dose independently is not a substitute for responding to the findings.

The metoprolol tartrate label lists severe bradycardia as a contraindication. This scenario is not a universal numerical hold rule. It also does not instruct a client to stop long-term beta-blocker treatment independently; the label warns about abrupt cessation of chronic therapy.

3. The exact dosage form matters

The third question specifies OXYCONTIN extended-release tablets. Crushing or dissolving that product can defeat its controlled release. Swallowing difficulty calls for assessment and an appropriate alternative plan, not an improvised change to the tablet or a substitution on the same schedule.

The OXYCONTIN label explains the formulation risk and discusses alternatives for swallowing difficulty. Check the exact product's instructions; do not extend one tablet's directions to every medicine or formulation.

Review the decision, not just the letter

After each question, write three lines:

  • The cue I noticed or missed: a route, a current finding or a formulation.
  • Why my tempting option was weaker: what risk or delay it left unresolved.
  • What I must verify next: the complete order, the client's condition or product-specific instructions.
